About The Position

The licensed Occupational Therapist is responsible for providing occupational therapy interventions to patients/clients in a Skilled Nursing setting as ordered by the referring physician. Responsibilities

  • Performs a comprehensive evaluation and reevaluation of patient in regards to occupational therapy needs upon referral in a timely manner using tests and measurements of strength, balance, endurance, range of motion, and activities of daily living as appropriate.
  • Based on the evaluation findings, a treatment plan will be established consisting of short and long term goals.
  • Reevaluations of patients will be performed at least every 30 days as stated by current state regulations.
  • The occupational therapist shall work with members of the interdisciplinary team in an effort to develop a treatment plan as appropriate.
  • Findings from the initial evaluation shall provide a platform for a discussion with the patient and/or family members in regards to the frequency of treatments and expectations in accordance with the prescription from the referring physician.
  • A qualified occupational therapist shall be present or readily available to offer supervision when an occupational therapist assistant or occupational therapy student administers treatment.
  • Ongoing conversations regarding findings of initial evaluation, progression and discharge will take with occupational therapist assistant as needed
  • Documentation will be completed accurately and in a timely manner in accordance with state laws.
  • An occupational therapist will prompt necessary discharge from therapy services when appropriate.
  • Short and long term goals have been met
  • Progress is no longer expected secondary to functional plateau, noncompliance, etc.
