Responsibilities: - Conduct assessments to evaluate patients' physical, emotional, and cognitive abilities - Develop individualized treatment plans based on assessment results - Provide occupational therapy interventions to improve patients' functional abilities - Monitor and document patients' progress and adjust treatment plans as needed - Collaborate with other healthcare professionals to coordinate patient care - Educate patients and their families on therapeutic techniques and strategies Requirements: - Master's degree in Occupational Therapy - Valid state license or certification as an Occupational Therapist - Previous experience in case management is preferred - Strong communication and interpersonal skills - Ability to work effectively in a team environment - Excellent problem-solving and critical-thinking abilities
