Occupational Therapist (OT) – Per Diem Weekends Only Willow Creek Health Care Center – Clovis, CA Pay Range: $68–$70/hour BOE Willow Creek Health Care Center is seeking a dedicated and compassionate Per Diem Occupational Therapist (OT) to join our rehabilitation team. General Purpose The Staff Occupational Therapist evaluates and treats residents, communicates with families, physicians, and interdisciplinary team members, and maintains accurate documentation of services in the medical record. This role also provides ongoing staff education and training. Essential Duties Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies. Develop, implement, and update individualized treatment plans to improve functional independence in ADLs. Supervise Occupational Therapy Assistants (COTAs), aides, and students in accordance with state practice guidelines. Assist nursing with training and support of Restorative Aides. Communicate regularly with physicians, nursing staff, interdisciplinary team, residents, and families regarding progress, goals, and discharge planning. Participate in care plan meetings, utilization review, and rehabilitation conferences as needed. Provide in-services and training programs for facility staff. Complete timely, accurate, and compliant documentation per regulatory and payer requirements. Maintain a safe treatment environment following infection control and safety protocols. Educate residents, families, and caregivers on therapy goals, home programs, safety techniques, and fall prevention. Recommend and facilitate ordering of adaptive equipment and assistive devices to support independence. Maintain confidentiality of resident and facility information. Report equipment concerns and ensure proper maintenance. Participate in quality assurance and performance improvement initiatives. Foster positive and professional relationships with residents, families, and team members. Supervisory Responsibilities Supervise Certified Occupational Therapy Assistants (COTAs), therapy aides, and students. Qualifications Education & Experience Bachelor’s Degree in Occupational Therapy required (Master’s or Doctorate preferred) Experience in a skilled nursing facility preferred Proficiency with documentation systems such as Net Health and PointClickCare Certificates & Licenses Current Occupational Therapist license in California (in good standing) CPR certification (if required) Must meet continuing education requirements per state regulations Skills & Abilities Strong clinical, communication, and interpersonal skills Ability to read and interpret technical and clinical documents Problem-solving and critical-thinking skills Ability to follow written and verbal instructions Physical Demands Frequent standing and walking; occasional sitting, bending, and lifting Ability to lift/move residents and equipment (up to 100 lbs with assistance) Effective communication and sensory awareness required for resident care Ability to assist with resident mobility and safe handling techniques Work Environment Skilled nursing facility setting Low to moderate noise level Team-oriented, fast-paced environment focused on quality outcomes Equal Employment Opportunity (EEO) Statement Willow Creek Health Care Center is an equal opportunity employer.
