Rock Creek Care Center is seeking a passionate and motivated Occupational Therapist (OT) to join our part-time therapy team. The Occupational Therapist evaluates and treats patients, develops individualized treatment plans, and collaborates closely with families, physicians, and the interdisciplinary care team. This role also provides leadership through staff education and supervision of Occupational Therapy Assistants to ensure high-quality, patient-centered care. Requirements

  • Bachelor’s degree in Occupational Therapy (Master’s or Doctorate preferred)
  • Current Occupational Therapist license in good standing with the State of California
  • Experience in skilled nursing or post-acute care preferred
  • Proficiency with documentation systems such as Casamba and PointClickCare
  • Strong communication, leadership, and teamwork skills
  • Must have consistent PRN availability

Responsibilities

  • Evaluate patients promptly and develop effective, physician-approved treatment plans
  • Provide skilled occupational therapy services in accordance with physician orders and best practices
  • Supervise and support COTAs, aides, and students in compliance with state practice acts
  • Document evaluations, treatments, and progress notes in accordance with regulatory and payer requirements
  • Collaborate with nursing and interdisciplinary teams to support patient progress and discharge planning
  • Educate patients, families, and caregivers on treatment plans and home programs
  • Recommend and facilitate durable medical equipment to promote patient independence
  • Participate in care conferences, utilization review, and staff in-services
  • Maintain therapy equipment and ensure a safe, effective treatment environment
