About The Position

Provide professional occupational therapy evaluation, treatment and follow up services to physician referred patients. Responsible for assessing patients and performing procedures consistent with or based on the age and developmental status of the patient to maximize the clinical outcome and safety of the patient.

Requirements

  • Education: Graduate from a school of Occupational Therapy with a Bachelor of Science or Masters Degree from an accredited college or university approved by the AOTA and the Council on Medical Education of the AMA.
  • Certification: Must be licensed or eligible for licensure as a Physical Therapist in the State of IL; CPR certification.
  • Experience: No clinical experience required; entry level position.

Responsibilities

  • Provides quality Occupational therapy services to patients including assessment, treatment intervention and discharge planning based on the age, psychological, educational and developmental status of the patient. Service provision may include direct, group, monitored and consultative approaches.
  • Communicates and collaborates with physicians, other health professionals and family members or caregivers to facilitate optimal patient care.
  • Prepares and maintains accurate patient records including evaluations, progress notes, discharge summaries and billing as required by Medicare, third party payers, and JCAHO/HFAP.
  • Assists in organizing and maintaining equipment, scheduling, etc.
  • Assures all patient care billing for treatment and equipment is completed according to policy

  • Assures co-signature on notes and evaluations if waiting on licensure exam results.
  • Reports on patient/resident progress at staffing and other conferences.
  • Anticipates the requirements and needs of others on a consistent basis.
  • Participates in bi-monthly occupational therapy staff meetings.
  • Participates in the clinical supervision of both Level I and Level II Occupational Therapy students, volunteers and high school students.
