About The Position

In this role, you will provide hands-on therapy services that help residents improve strength, mobility, coordination, and performance in daily activities. You’ll collaborate closely with residents, families, and care teams to support safe, effective, and person-centered therapy.

Requirements

Must be a registered Occupational Therapist
Licensed and certified Occupational Therapist in the state of Minnesota
Strong communication, organization, and interpersonal skills
Ability to work effectively with residents, families, and interdisciplinary teams
Ability to lift and carry up to 50 pounds
Assist residents with transfers and mobility exercises
Frequent standing, walking, bending, kneeling, and reaching
Ability to work in varied physical positions throughout the workday

Nice To Haves

Experience in skilled nursing, long-term care, or senior living settings
Knowledge of adaptive equipment and assistive technology
Familiarity with electronic medical record (EMR) systems

Responsibilities

Implement individualized treatment plans
Guide residents through therapeutic exercises, activities, and functional interventions
Provide training on adaptive equipment and assistive devices to residents and caregivers
Monitor resident progress and communicate changes to the supervising Occupational Therapist
Educate residents and families on techniques to support continued function and independence
Collaborate with therapy, nursing, and interdisciplinary teams to ensure coordinated care
Participate in care conferences and support discharge planning
Integrate therapy interventions into residents’ daily routines
Ensure compliance with state and federal regulations, including safety and infection control standards
Maintain accurate and timely documentation within the EMR system
