Equal Opportunity Employer. Position Summary We are hiring an experienced Home Health Occupational Therapist (OT) to provide patient-centered therapy in the home setting. This position offers autonomy, flexible scheduling, and strong interdisciplinary collaboration. Requirements

• Graduate of an accredited Occupational Therapy program
• Active Maryland OT license (required)
• Minimum 3 years clinical experience (home health preferred)
• Strong assessment and care planning skills
• EMR experience preferred (WellSky/Kinnser or Homecare Homebase)
• Valid driver’s license and reliable transportation

Responsibilities

• Complete in-home OT evaluations and reassessments
• Develop individualized treatment plans
• Deliver skilled interventions to improve ADLs and functional independence
• Educate patients and caregivers on safety and home exercise programs
• Track progress and adjust care plans accordingly
• Document visits accurately and timely in EMR
• Collaborate with physicians, nurses, and care team members
• Supervise Certified Occupational Therapy Assistants (COTAs)
• Maintain Medicare, HIPAA, and agency compliance
