Position Overview Reporting to the Home Care Director, the Granite VNA Occupational Therapist (OT) delivers age-appropriate care to the patient in the home setting utilizing standard therapy practice: assessment, planning, intervention, implementation and evaluation within scope of practice defined by the NH Occupational Therapy Board. OTs will effectively interact with patients, caregivers and other healthcare team members while maintaining all standards of professional therapy. OTs will supervise Certified Occupational Therapist Assistants assigned to patients.
