Requirements

  • Active Colorado Occupational Therapist license (required)
  • Home health experience preferred, but not required
  • Strong clinical judgment and comfort working independently
  • Ability to provide guidance and collaboration to a COTA within scope of practice
  • Excellent communication and time-management skills
  • Reliable vehicle, valid driver's license, and auto insurance (required)
  • A genuine passion for helping people maintain independence and dignity

Nice To Haves

  • Home health experience preferred, but not required

Responsibilities

  • Deliver individualized OT services in patients' homes
  • Develop functional, patient-centered treatment plans
  • Complete home safety evaluations and recommend adaptive equipment
  • Oversee and collaborate with a qualified, competent COTA, providing clinical direction and support as appropriate
  • Partner with an interdisciplinary care team (nursing, PT, caregivers, families)
  • Document care efficiently with full EMR training and support
  • Travel locally within Mesa County (mileage and drive time reimbursed)
