About The Position

As an Occupational Therapist, you will play a critical role in helping individuals of all ages regain and improve their ability to perform daily activities and enhance their quality of life. You will assess patients' physical, emotional, and cognitive needs to develop personalized treatment plans that promote independence and functional recovery. Your work will involve collaborating with multidisciplinary teams to address complex rehabilitation challenges, particularly in areas such as geriatric care, cerebral palsy, orthopedic rehabilitation, and neurological conditions like stroke and traumatic brain injury. You will utilize evidence-based therapeutic techniques, including sensory integration and physical therapy modalities, to support patients' progress. Ultimately, your efforts will empower patients to overcome barriers and achieve meaningful participation in their home, work, and community environments.

Requirements

  • Valid Occupational Therapy License
  • 1 year of occupational therapy experience preferred
  • CPR certification
  • Valid Driver’s License & Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Perform clinical assessment; identifies patient needs and appropriateness of patient.
  • Documents, prepares, revises, and completes a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.
  • Accepts clinical assignments that are consistent with education and competence to care for patients/clients.
  • Participates in the IDG care planning process.
  • Apply concepts of infection control and standard precautions in coordination/performing patient care activities to prevent contamination and transmission of disease.
  • Assess and facilitate through direct therapeutic intervention the patient's ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patient’s ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Evaluate patient occupational performance in the areas of self-care, work, leisure and rest. Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior. Assess physical, social and cultural components of the patient’s occupational environment.
  • Meets mandatory continuing education requirements of the Agency and the licensing board.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
