About The Position

The Occupational Therapist (OT) in an acute inpatient rehabilitation setting evaluates and treats patients recovering from illness, injury, or surgery to improve functional independence in activities of daily living (ADLs), mobility, and cognitive/perceptual skills. The OT utilizes GG (Functional Independence Measure – FIM/GG) scoring to assess patient progress and document functional outcomes in compliance with CMS (Centers for Medicare & Medicaid Services) and other regulatory requirements.

Requirements

  • Master’s/Doctorate in OT (ACOTE-accredited).
  • Current state OT license; NBCOT certification.
  • Acute care/IPR experience preferred (new grads with Level II fieldwork in these settings considered).
  • Familiarity with GG scoring (critical for IPR roles).
  • ICU/acute care competency (ventilators, lines/tubes, hemodynamic monitoring).
  • Knowledge of IRF-PAI & CMS regulations (for IPR).

Responsibilities

  • Conduct bedside evaluations in acute care for medically fragile patients (ICU, step-down, medical/surgical units).
  • Perform comprehensive rehab assessments in IPR, focusing on ADLs, mobility, cognition, and discharge readiness.
  • Administer GG scoring (Section GG) for IPR patients per CMS guidelines (e.g., GG0130: Eating, GG0170: Sitting to Standing).
  • Use standardized assessments (e.g., MoCA, MMT, FIM) in both settings.
  • Early mobilization of ICU/post-surgical patients to prevent deconditioning.
  • Energy conservation & compensatory strategies for patients with cardiac/pulmonary conditions.
  • Dysphagia & adaptive feeding techniques (collaborating with SLPs).
  • Splinting & edema management for post-op orthopedic/neuro patients.
  • High-intensity ADL retraining (dressing, bathing, toileting).
  • Neuromuscular re-education for stroke/TBI recovery.
  • Cognitive-perceptual rehab (memory, attention, executive function).
  • Home assessments & caregiver training pre-discharge.
  • Accurately complete Section GG functional scoring at admission, discharge, and per IRF-PAI guidelines.
  • Document medically necessary skilled OT services (e.g., progress notes, DC planning).
  • Ensure compliance with CMS guidelines (Medicare Part A, IRF coverage criteria).
  • Recommend DME, home health, or SNF placement based on patient stability.
  • Coordinate with case managers for safe community reintegration.
  • Educate patients/families on post-discharge care in both settings.
  • Follow hospital protocols (e.g., isolation precautions, code response).
  • Participate in interdisciplinary rounds (especially in ICU/acute care).
  • Maintain state licensure, NBCOT, BLS/ACLS (required in many acute care settings).
