Compensation Package: Compensation is piece-rate, paid per completed patient visit Per Visit: $75 – $150 per completed visit (good-faith estimate upon hire based on type of visit) Mileage Reimbursement: $0.72 per mile Benefits Flexible scheduling Supportive team environment Job Summary As an Occupational Therapist (PT) with GrandCare East, you will be responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the physician’s plan of care.

Requirements

  • Valid CA OT License
  • Possesses and maintains current CPR certification.
  • 2+ years of clinical experience as an occupational therapist
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Responsibilities

  • Evaluate functional abilities and home environment, assessing areas such as muscle function, endurance, coordination, communication, self-care skills, work capacity, and safety hazards; identify equipment and adaptive needs.
  • Develop and implement individualized treatment plans, set functional goals, and communicate the plan of care with physicians and interdisciplinary team members.
  • Teach, retrain, and support patients in performing daily living activities, including use of adaptive equipment, splints, prosthetics, and orthotics to improve independence.
  • Document clinical care, maintain communication with care team members, participate in case conferences, and report changes in condition; support discharge planning and referrals for additional services.
  • Supervise support staff, including Certified Occupational Therapy Assistants and, when OT is the only skilled service, evaluate and supervise home health aide care every two weeks.
