Diablo Valley Post Acute, a premier provider of rehabilitation and long-term care in Contra Costa County, is seeking a compassionate Occupational Therapist (OTR) to join our dynamic and passionate team. Requirements

  • Valid Occupational Therapy license in the state of California.
  • New graduates are welcome to apply.

Responsibilities

  • Develop and implement effective, physician-approved treatment plans for patients.
  • Provide direct patient care and supervise Occupational Therapy Assistants in accordance with state practice acts.
  • Train restorative aides to ensure continuity of care.
  • Communicate patient progress, problems, and plans with the interdisciplinary healthcare team.
  • Document evaluations, progress notes, and other required information in a timely and compliant manner.
  • Educate patients, families, and nursing staff on maintenance programs and caregiving techniques.
  • Facilitate discharge planning and recommend necessary durable medical equipment.
  • Maintain departmental equipment in good working order and report any issues.
