About The Position

As an Occupational Therapist at San Bruno Skilled Nursing, you will be responsible for developing, implementing, and documenting treatment plans for patients under physician orders. You will collaborate closely with other healthcare team members and supervise Occupational Therapy Assistants in providing patient care. Responsibilities

  • Develop treatment plans for patients and obtain approval from referring physicians.
  • Implement treatment sessions according to the physician-approved plan.
  • Train Restorative Aides in the nursing department.
  • Supervise Occupational Therapy Assistants in direct patient care activities.
  • Communicate with your supervisor and other team members regarding patient progress and treatment plans.
  • Attend patient care, utilization review, and rehabilitation conferences as necessary.
  • Participate in in-service training programs for other staff.
  • Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in compliance with all regulations.
  • Update and document treatment changes following facility policies.
  • Teach patients' families or nursing staff on maintenance programs and caregiver training for discharge.
  • Participate in discharge planning.
  • Recommend and order necessary durable medical equipment for patients, such as walkers, canes, elevated toilet seats, and grab bars.
  • Report any issues with department equipment.
