Pleasant Hill Post Acute is seeking a dedicated and skilled Occupational Therapist (OTR) to join our premier in-house therapy department. We are looking for a clinician who is passionate about geriatric rehabilitation and helping residents reclaim their independence in activities of daily living. Requirements

  • A current and valid California Occupational Therapist (OT) License.
  • A compassionate, "resident-first" mindset with a focus on functional outcomes.
  • Strong leadership skills and the ability to work effectively as part of a multi-disciplinary team.

Responsibilities

  • Design the Recovery Path: Develop comprehensive, evidence-based treatment plans and secure physician approvals to help residents return to their highest level of function.
  • Clinical Leadership: Supervise Occupational Therapy Assistants (COTAs) and mentor the nursing department through the training of Restorative Aides.
  • Functional Rehabilitation: Provide direct treatment focused on mobility, self-care, and cognitive function according to personalized care plans.
  • Interdisciplinary Collaboration: Actively participate in Patient Care Conferences, Utilization Reviews, and Rehabilitation meetings to ensure holistic care.
  • Meticulous Documentation: Maintain precise evaluations, daily notes, and recertification records in compliance with the OT Board and third-party payer standards.
  • Discharge & Equipment Strategy: Facilitate safe transitions home by instructing families in maintenance programs and recommending essential durable medical equipment (grab bars, adaptive tools, etc.).
  • Professional Stewardship: Build meaningful relationships with residents and families while ensuring all department equipment is maintained for safety and excellence.
