We are seeking an experienced Home Health Occupational Therapist to deliver high-quality, patient-centered care in the home setting. This role offers autonomy, flexibility, and collaboration with an interdisciplinary care team.

Requirements

• Graduate of an accredited OT program
• Active Pennsylvania OT license
• Minimum 3 years of clinical experience (home health preferred)
• Strong assessment, clinical reasoning, and documentation skills
• EMR experience preferred (Kinnser/WellSky or Homecare Homebase)
• Reliable transportation and valid driver’s license

Nice To Haves

• COTA supervision experience preferred

Responsibilities

• Perform comprehensive OT evaluations in patients’ homes
• Develop and implement individualized treatment plans
• Provide skilled interventions to improve function, mobility, and safety
• Educate patients and caregivers on exercises, safety, and injury prevention
• Monitor progress and adjust care plans as needed
• Document all visits accurately and timely in the EMR
• Collaborate with physicians, nurses, and care team members
• Maintain compliance with Medicare, HIPAA, and agency standards
