Requirements
- Bachelor’s or Master’s Degree in Occupational Therapy from an accredited program
- Active Texas Occupational Therapist (OT) license
- Home health occupational therapy experience preferred (1+ year required)
- Experience with OASIS documentation preferred
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Knowledge of home health regulations and Medicare guidelines preferred
- Valid driver’s license, reliable transportation, and current auto insurance
Responsibilities
- Perform home health occupational therapy evaluations, including OASIS assessments
- Develop and update individualized Plans of Care (POC)
- Provide skilled home health occupational therapy for orthopedic, neurological, cognitive, and post-surgical conditions
- Address ADLs, IADLs, upper extremity function, coordination, strength, balance, and functional mobility
- Implement interventions for fall prevention, home safety, and environmental modifications
- Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
- Fabricate splints and recommend assistive devices as appropriate
- Complete accurate and timely documentation in the EMR
- Communicate with physicians and interdisciplinary care teams
- Supervise COTAs and home health aides, as applicable
- Follow all home health regulations, infection control, and safety standards
