Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. Our team is growing and looking for a dedicated: Occupational Therapist ( OT) Shifts Available: Full Time, Part Time & PRN Service Areas: Northeast (Crosby/Highlands), Northwest (Humble,Atascocita,Kingwood), Conroe, Lake Jackson Responsibilities: Facilitate rehabilitation of patients with emotional, physical, or mental disabilities. Develop and implement occupational therapy care plans. Coordinate care with home healthcare team. Evaluate therapy needs and monitor and report patient progress and outcomes. Educate patients and family members. Prepare timely, complete, and accurate clinical notes. Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members. Qualifications: Occupational therapist in the state of Texas. Home health experience highly preferred. Knowledge of home health regulations and general healthcare compliance. Able to handle physical workload. Good driving record. Experience with HCHB EMR (preferred but not required). Effective communication and people skills.
