About The Position

Position Purpose: Join Trinity Health At Home as a Home Care Occupational Therapist ( OT ) and make a meaningful impact by delivering compassionate, high-quality therapy services to clients in their homes. You'll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions. Requirements

  • Graduate of an accredited Occupational Therapy program.
  • NBCOT certification and state licensure/registration.
  • Minimum one year of experience or completion of a 6-8 week preceptorship.
  • Valid driver's license and reliable transportation.
  • Commitment to ethical standards and continuous learning.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Assess clients' functional status and develop individualized treatment plans in collaboration with physicians.
  • Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
  • Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
  • Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
  • Coordinate care with interdisciplinary teams and participate in case conferences.
  • Document all care activities accurately using point-of-care systems.
