About The Position

You will work with elderly participants over months and years, helping them maintain independence in their homes and communities through adaptive strategies, home modifications, cognitive interventions, and caregiver education. Unlike acute settings where you see patients briefly, PACE allows you to build genuine therapeutic relationships and witness the long-term impact of your clinical expertise. What You Will Do You will provide occupational therapy evaluations and ongoing treatment for PACE participants, focusing on ADL training, fall prevention, home safety assessments, cognitive maintenance, adaptive equipment recommendations, and caregiver education. You will collaborate with an interdisciplinary team in a community-based model that prioritizes holistic, person-centered care.

Requirements

  • Master's or Doctoral degree in Occupational Therapy
  • Current or eligible for Pennsylvania State OT license
  • NBCOT certification
  • Interest in geriatric care and community health
