Requirements

  • Graduate of an accredited Occupational Therapy program
  • Current, unrestricted OT license in the state of practice
  • Minimum 1 year of clinical experience
  • Ability and willingness to travel within the branch coverage area (up to 50%)
  • Reliable vehicle, valid driver’s license, and current auto insurance
  • Ability to sit, stand, bend, lift, and move intermittently throughout the workday

Responsibilities

  • Evaluate patients to determine occupational therapy needs, rehabilitation potential, and develop individualized Plans of Care
  • Perform initial evaluations, routine visits, and re-evaluations in compliance with OASIS and PPS guidelines
  • Ensure continuity of care and appropriate discharge planning
  • Communicate significant findings or changes in patient condition to supervisors, physicians, and care team members
  • Implement patient safety plans using patient, family, and community resources
  • Verify and follow physician orders, assessment data, and established clinical standards
  • Write physician orders for additional visits or changes to the Plan of Care per agency policy
  • Incorporate patient goals into therapy interventions and document progress clearly and accurately
  • Provide occupational therapy services consistent with best practices and physician certification
  • Educate patients and caregivers on activities of daily living, adaptive equipment, sensory integration, and fine motor skills
  • Assist with securing necessary equipment, including fabrication of custom splints when appropriate
  • Instruct patients and caregivers on home therapy programs, modifying as needed
  • Counsel patients and families on therapy-related needs
  • Supervise and provide clinical direction to COTAs, HHAs, and other clinical staff to ensure quality and consistency of care
