About The Position
LP Therapy is seeking a licensed Occupational Therapist (OT) to provide evaluation and treatment services in a Long-Term Care setting, helping patients regain independence and improve daily functioning.
Requirements
- Bachelor's degree in Occupational Therapy
- current state OT license
- LTC experience preferred
- strong knowledge of Medicare documentation
- excellent communication and organizational skills

Responsibilities
- review medical records
- evaluate patient abilities and needs
- develop and implement individualized treatment plans
- provide ADL training, adaptive equipment, and environmental modifications
- document progress per Medicare guidelines
- collaborate with interdisciplinary teams
- supervise COTAs as assigned
- support patient satisfaction and quality outcomes
