We are seeking an experienced Home Health Occupational Therapist to provide skilled, patient-centered therapy services in patients’ homes. This role offers clinical autonomy while collaborating with an interdisciplinary care team to deliver high-quality outcomes.

Requirements

• Graduate of an accredited Occupational Therapy program
• Active DC Occupational Therapist license
• Minimum 3 years OT experience (home health or SNF preferred)
• Strong clinical judgment and communication skills
• Reliable transportation and valid driver’s license

Nice To Haves

• EMR experience preferred (WellSky/Kinnser or Homecare Homebase)

Responsibilities

• Perform comprehensive OT evaluations and functional assessments
• Develop and implement individualized treatment plans
• Provide skilled OT interventions to improve ADLs, mobility, and safety
• Educate patients and caregivers on exercises, safety, and injury prevention
• Monitor progress and modify treatment plans as needed
• Document visits accurately and timely in EMR
• Collaborate with physicians, nurses, and care teams
• Maintain compliance with HIPAA, Medicare, and agency policies
• Supervise COTAs as assigned
