We are seeking an experienced Home Health Occupational Therapist (OT) to provide skilled occupational therapy services in patient homes. This role focuses on improving ADLs, functional independence, safety, and quality of life while collaborating with an interdisciplinary care team.

Requirements

• Licensed Occupational Therapist in Washington, DC
• Minimum 3 years OT experience (home health preferred)
• Strong clinical and documentation skills
• EMR experience (WellSky/Kinnser or Homecare Homebase preferred)
• Reliable transportation and valid driver’s license

Responsibilities

• Perform OT evaluations, treatments, and reassessments
• Develop and implement treatment plans per physician orders
• Improve ADLs, mobility, balance, strength, and function
• Educate patients and caregivers on safety and home programs
• Document care accurately in EMR systems
• Collaborate with physicians, nurses, and clinical team
• Supervise COTA
• Maintain compliance with Medicare and HIPAA guidelines
