We are seeking an experienced Home Health Occupational Therapist who is passionate about patient-centered care. This role offers autonomy, flexibility, and collaboration with a strong interdisciplinary team to deliver high-quality therapy in patients’ homes.

Requirements

• Graduate of an accredited OT program
• Current Virginia OT license required
• Minimum 3 years of clinical experience (home health preferred)
• Strong assessment, documentation, and care-planning skills
• Reliable transportation & valid driver’s license

Nice To Haves

• EMR experience preferred (Kinnser/WellSky or Homecare Homebase a plus)

Responsibilities

• Perform comprehensive occupational therapy evaluations
• Develop and implement individualized treatment plans
• Provide skilled OT interventions to improve daily functioning
• Educate patients and caregivers on exercises, safety, and prevention
• Monitor progress and adjust care plans as needed
• Document all care accurately and timely in the EMR
• Collaborate with physicians and care teams
• Ensure compliance with Medicare, HIPAA, and regulatory standards
• Supervise Certified Occupational Therapy Assistants (COTAs)
