Requirements

  • Graduate of an accredited Occupational Therapy program
  • Current, unrestricted Occupational Therapist license in the state of practice
  • Minimum of one (1) year of clinical experience preferred
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to sit, stand, bend, lift, and move intermittently throughout the day
  • Physically active, home-based work environment
  • Full range of motion, including lifting, transferring, and assisting patients
  • Ability to lift 50–100 pounds
  • Ability to travel within the assigned coverage area (up to 50%)
  • Potential for occasional irregular or on-call hours

Responsibilities

  • Evaluate patients to determine occupational therapy needs and develop individualized plans of care
  • Complete initial evaluations and ongoing reassessments in compliance with OASIS and PPS guidelines
  • Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
  • Implement patient-centered interventions focused on activities of daily living, adaptive equipment, sensory integration, and fine motor coordination
  • Educate patients and caregivers on home programs and therapy techniques
  • Assist with securing necessary equipment, including fabrication of custom splints when needed
  • Provide supervision and clinical direction to COTAs, HHAs, and other clinical staff as applicable
  • Maintain accurate, timely documentation in accordance with agency and regulatory standards
