Requirements

• Florida OT license.
• 1+ year clinical experience (home health preferred).
• Strong communication, documentation, and organizational skills.
• Reliable transportation.
• Florida COTA license.

Responsibilities

• Deliver in -home occupational therapy per physician orders.
• Develop treatment plans to improve daily living skills.
• Educate patients and caregivers on exercises and mobility.
• Document visits in Synergy EMR.
• Collaborate with interdisciplinary team members.
• Support OT in patient care plans.
• Document progress accurately.
• Strong documentation skills.
• Document visits in Synergy EMR.
