Requirements

Arizona Level One IVP Fingerprint Clearance Card
AZ OT or COTA License (Arizona Board of OT Examiners)
CPR/First Aid
AHCCCS and NPI number

Responsibilities

Provide evidence-based occupational therapy services to help students access and succeed in their educational environment.
Collaborate daily with teachers, families, and other school staff to support student success.
OT only: Conduct evaluations
Write IEP/MET documentation, and monitor student progress.
Maintain compliance with state and federal guidelines while fostering a positive, team-oriented atmosphere.
