The Occupational Therapist is responsible for implementation of standards of care for Occupational Therapy Services. Core Competencies Evaluates patients to obtain and interpret data necessary for treatment planning and implementation. Analyzes evaluation results to identify the patients major problems, establishes goals with the patient, and develops and adjusts treatment programs. Performs Occupational Therapy treatments based on evaluation results, aimed at increasing a patient's functional level. Demonstrates knowledge of clinical norms for all age groups as applicable to job functions. Maintains clinical records, clinical notes, and reports to the physician any changes in the patients condition. Attends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization. Identifies patient and family/caregiver needs for home health services and refers as appropriate. Department Specific Competencies Establishes I will statements at annual performance evaluation and implements the statements. Supervises and documents the home health aide plan of care every 2 weeks.
