Occupational Therapist Home Health Part Time
About The Position
At Qualicare Home Health, we deliver exceptional Home Care and Home Health services with compassion, respect, and professionalism. Our San Ramon-based team is growing, and we’re seeking a dedicated Occupational Therapist (OT) to help clients regain independence, improve daily living skills, and achieve their highest possible quality of life. In this role, you’ll design and implement individualized therapy plans, work closely with caregivers and clinical staff, and ensure each client receives seamless, personalized care tailored to their unique needs. Position Summary We are seeking a skilled and compassionate Occupational Therapist (OT) to join our home health team. In this role, you will assess patient needs, design individualized care plans, and deliver therapeutic interventions that improve daily living skills and safety in the home.
Requirements
- Active, unrestricted California Occupational Therapist (OT) license
- Minimum 1 year of clinical experience (home health or geriatrics preferred)
- Current CPR certification
- Reliable transportation and valid CA driver’s license
- EMR documentation experience preferred
- Strong communication, adaptability, and problem-solving skills
Responsibilities
- Perform initial evaluations to assess ADL/IADL capabilities and home safety risks
- Develop goal-oriented treatment plans to enhance independence in self-care and daily tasks
- Address physical, sensory, emotional, and cognitive barriers to function
- Educate patients and caregivers on adaptive techniques and assistive device use
- Recommend home modifications to reduce risks and improve accessibility
- Document care accurately in the EMR (WellSky) and maintain compliance with regulations
- Collaborate with PTs, RNs, and interdisciplinary team members for coordinated care
- Participate in team meetings, QA initiatives, and case conferences
