As a nonprofit, mission-driven agency with over 100 years of community care, we're seeking a compassionate, skilled Occupational Therapist to join our dynamic team. In this field-based role, you'll play a vital part in supporting patients of all ages to regain skills, navigate daily activities, and adapt their home environments for safety and success. This position will cover towns in Southern Fairfield County, including Norwalk, Stamford and Greenwich. What You'll Do As a Home Health Occupational Therapist , you will:

Requirements

  • Bachelor's (or higher) degree in Occupational Therapy from an accredited program
  • Current Connecticut OT license in good standing
  • 2+ years of clinical experience (Home health preferred but not required)
  • Strong knowledge of home care regulations, documentation, and assistive technology
  • Excellent communication, time management, and problem-solving skills
  • Ability to work independently while collaborating across a care team
  • Valid driver's license and reliable transportation for travel throughout Southern Fairfield County

Responsibilities

  • Perform comprehensive functional assessments and contribute to interdisciplinary care planning with the RN, PT, patient, family, and physician.
  • Develop and implement patient-centered therapy plans focused on maximizing independence in daily living.
  • Conduct OASIS assessments accurately and in a timely manner.
  • Supervise and collaborate with COTAs and Home Health Aides , ensuring quality care and compliance.
  • Educate patients and caregivers on safe techniques, assistive equipment, and energy conservation strategies.
  • Support patients in managing chronic conditions and adapting their routines to live with purpose and autonomy.
  • Make timely and appropriate referrals for additional home health services or higher levels of care.
  • Accurately document assessments, progress, and care coordination in our electronic medical record system.
  • Participate in case conferences, clinical team huddles, and agency-wide quality improvement efforts.
