About The Position

Adventist Health Columbia Gorge is a compassionate, faith-based healthcare organization dedicated to providing exceptional, patient-centered care to our community. Our Home Health department offers personalized care to patients in the comfort of their homes, ensuring they maintain the highest quality of life. We are seeking a skilled Occupational Therapist to join our team and help deliver expert rehabilitation services to those in need. We are looking for an enthusiastic Occupational Therapist to join our Home Health team in providing in-home therapy services to patients recovering from illness, injury, or surgery. In this part-time role (30 hours per week), you will work directly with patients in their homes, helping them regain independence, improve daily functioning, and enhance overall well-being.

Requirements

• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Current Occupational Therapist (OT) license in the state of Oregon.
• Experience in Home Health or outpatient therapy preferred, but not required.
• Knowledge of home safety evaluations and adaptive equipment.
• Strong interpersonal and communication skills to engage patients and families.
• Ability to work independently with minimal supervision and collaborate effectively with a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Reliable transportation and ability to travel to patient homes within the service area.

Responsibilities

• Assess patients' physical, cognitive, and emotional abilities to identify areas of need.
• Develop and implement individualized therapy plans focused on improving the ability to perform daily activities.
• Provide home safety assessments and recommend modifications to ensure patients' environments are safe and accessible.
• Collaborate with patients, families, caregivers, and the interdisciplinary Home Health team to provide comprehensive care.
• Educate patients and their families on proper techniques, adaptive equipment, and self-care strategies to improve daily living.
• Maintain accurate patient records, document progress, and communicate with the care team to adjust therapy plans as needed.
• Promote health education and offer resources to support patients' ongoing recovery and well-being.
