About The Position

As a Home Health Occupational Therapist, you'll play a vital role in empowering patients to achieve their highest level of function and independence—all within the comfort and familiarity of their own homes. Utilizing your expertise in physical assessment, therapeutic interventions, and patient education, you'll help individuals regain mobility, improve their ability to perform daily living activities, rebuild confidence, and enhance their overall quality of life. In this role, you'll work closely with physicians and the interdisciplinary home health team to develop and implement personalized care plans that support recovery, reduce hospital readmissions, and promote long-term wellness. You'll be responsible for the evaluation, planning, direction, and administration of occupational therapy services as prescribed by a licensed physician, nurse practitioner, or physician assistant. You will guide patients in reaching their maximum potential through tailored treatment strategies, while coordinating, delegating, and supervising tasks assigned to supportive staff to ensure the highest standard of care. As a valued member of the home health team, you will report directly to the Home Health Director. Requirements

  • Current state Occupational Therapy licensure.
  • Graduate of an approved school of Occupational Therapy or accredited college/university with a certificate in Occupational Therapy.
  • Previous experience in all aspects of occupational therapy preferred.
  • Current BCLS certification.
  • Ability to read and communicate effectively in English.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Knowledge of physical medicine, occupational therapy modalities, anatomy and physiology.
  • Knowledge of occupational therapy equipment operation, maintenance, and repairs.
