About The Position

We are hiring a COTA for our Home Health Visits in Broken Arrow/Tulsa! The Occupational Therapy Assistant in Home Health is responsible for providing occupational therapy to patients in their homes to restore them to their fullest physical ability. The Occupational Therapy Assistant performs his/her duties in accordance with policies and procedures and the established plan of care.

Requirements

  • Current OTA licensure in the state of OK
  • Current CPR certification
  • Current driver's license and vehicle insurance, and access to a dependable vehicle

Responsibilities

  • Under the supervision of an Occupational Therapist, is at all times responsible to follow all state specific laws governing the provision of occupational therapy in home care.
  • Follow the treatment set only as defined by the Occupational Therapist.
  • Perform only those procedures that he/she is qualified and trained to perform.
  • Make all recommendations for changes in the plan of care to the supervising Occupational Therapist.
  • Following the plan of care, instruct and aid patients in evidence based treatment within the scope of the occupational therapy assistant and according to the state laws governing occupational therapy assistants.
  • Observe, record, and report to the supervising OT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
  • Coordinate care with the other members of the healthcare team as appropriate.
  • Instruct patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the occupational therapy assistant.
  • Visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed.
  • According to Plan of Treatment; complete a progress note for each visit and submit progress notes per LHC policy.
  • Adhere to LHC Therapy Standards of Practice in provision of service to all patients.
  • Participate in staff conferences and committees as requested by the agency.
  • Schedule flexibility is needed in this role to ensure we provide care to the patient as needed.
  • Participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients.
  • All other duties as assigned.
