About The Position

The Observation Unit Advanced Practice Provider utilizes a patient-centered coordinated care model, demonstrating competenciesin leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN or PAworks collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Theposition will cover multiple cardiac service lines where needed (Observation, HF/MI, and Cath lab). -Minimum of 3 years of experience in an acute care setting (hospital-based experience) required-Cardiology experience strongly preferred-Current APP licensure and certification (ACNP or PA) in good standing-Strong clinical assessment and decision-making skills-Excellent communication skills, proven team player, ability to multitask and prioritize, and proactive and creative problem-solver with afocus on process improvement, care coordination, and patient-centered solutions. Requirements

  • Minimum of 3 years of experience in an acute care setting (hospital-based experience) required
  • Current APP licensure and certification (ACNP or PA) in good standing
  • Strong clinical assessment and decision-making skills
  • Excellent communication skills, proven team player, ability to multitask and prioritize, and proactive and creative problem-solver with afocus on process improvement, care coordination, and patient-centered solutions.
  • Graduate of an NLN/AACN accredited nursing program.
  • Master's or Doctoratedegree from an accredited Nurse Practitioner program.
  • Maintains clinical competencies through continuing education and clinicalpractice.
  • Experience in a relevant patient care setting (or as regulations dictate).
  • National Provider Identifier (NPI).
  • Prescriptiveauthority through CDS and DEA registration according to New Jersey state law.
  • Collaborative agreement with physicians on staff.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/orGoogle Suite platforms.
  • NJState Professional Registered Nurse License.
  • New Jersey Advance Practice Nurse License.
  • BLS AHA Basic Health Care Life Support HCPCertification.
  • Board certified by AANP or ANCC.
  • Successful completion of a Physician Assistant program accredited by the AMA Council on Medical Education.
  • Current, unrestricted New Jersey Physician Assistant License.
  • Certification by the National Commission on Certification of Physician Assistants (NCCPA).
  • Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Excellent written and verbal communication skills.
  • Proficiency with computer platforms, including Microsoft Office and/or Google Suite.

Nice To Haves

  • Cardiology experience strongly preferred
  • Advanced Cardiac Life Support Certification.
  • Current Advanced Cardiac Life Support (ACLS) certification.

Responsibilities

  • Performs and documents accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender,and clinical problem.
  • Orders, performs, and interprets laboratory tests and diagnostic studies/procedures in collaboration with physiciansand consultants as appropriate.
  • Manages general medical and surgical conditions based on comprehensive knowledge of etiologies, riskfactors, pathophysiology, and the indications and contraindications of pharmacologic agents and other treatment modalities.
  • Prescribesmedication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education asneeded.
  • Seeks opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition (locally, nationally, andinternationally) through conferences, publications, and presentations.
  • Provides care that is patient- and family-centered, compassionate,and effective for the promotion of health and treatment of disease.
  • Practices as a healthcare provider according to the rules andregulations of the Medical Staff and bylaws as outlined in the delineation of privileges.
  • Adheres to HMH nursing protocols, standards ofcare, and disease-specific quality indicators.

  • Facilitates consistent, coordinated careand clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds.
  • Serves asan advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate.
  • Serves as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities.
  • Participates in organizational and departmental meetings, committees, and workgroups as necessary.
  • Demonstrates the highest level ofaccountability for professional practice and adheres to ethical principles regarding patient confidentiality, informed consent, and adverseoutcomes.
  • Collaborates with medical staff to support nursing-led protocols.
