About The Position

Requirements

• Graduate of an accredited school of nursing.
• RN-Registered Nurse of California certification.
• BLS-Basic Life Support Healthcare Provider certification.
• 2 years of recent relevant experience.
• Professional knowledge of clinical nursing protocols and regulations.
• Advanced clinical knowledge of medical diagnoses, procedures, and treatments.
• Solid analytical and project management skills.
• Strong communication, interpersonal, and interviewing skills.
• Proficient computer skills, including Microsoft Office Suite and EMR experience.
• Ability to work independently and as part of a multidisciplinary team.
• Exceptional attention to detail and organizational skills.

Responsibilities

• Aid patients in obtaining the correct level of care with the appropriate provider.
• Provide advanced clinical telephone support to Sutter Health patients and other callers.
• Assess patients' needs and appropriately disposition cases.
• Collaborate with clinic and hospital-based providers to renew electronic prescriptions.
• Identify hospital and community resources, consultations, and referrals.
• Perform nursing follow-up activities.
• Assist physician partners with message management and communications within the EMR system.
• Provide limited patient care in an outpatient setting.
