About The Position

The Oak Ridge Healthlink Care Coordinator plays a vital role in providing case coordination and support to individuals with severe and persistent mental illness (SPMI). This position involves working closely with clients to develop and implement care plans, ensuring they receive the necessary support and resources to improve their quality of life. The coordinator will engage with various agencies to facilitate cooperation and consistency in care, while also providing crisis intervention and transportation as needed. The role emphasizes a team-based approach to health care services, focusing on recovery and self-management of mental health issues.

Requirements

  • Bachelor's degree in a health-related field such as counseling, psychology, or social work.
  • Experience working with individuals with developmental disabilities or mental health issues.
  • Bilingual skills are preferred.
  • Basic computer skills for timekeeping and electronic medical records entry.
  • Valid driver's license with F-Endorsement and a dependable vehicle for conducting Center business.

Nice To Haves

  • Experience in cultural diversity counseling.
  • Knowledge of behavioral health and treatment of medical disorders and mental illness.

Responsibilities

  • Provide a minimum of 1-2 contacts per month with each client on the caseload.
  • Interface with agencies to increase cooperation and consistency between home, school, work, community, and health providers.
  • Provide crisis intervention and emergency services as needed.
  • Triage clients requiring in-person community visits and coordinate with the Treatment Team weekly.
  • Provide transportation to clients to meet the goals of their Care Plan.
  • Maintain appropriate chart records and complete documentation in a timely manner.
  • Participate in treatment team meetings with RN Care Coordinator.
  • Provide therapeutic support and crisis intervention as needed.
