About The Position

The Patient Navigator Coordinator at Henry Street Settlement is responsible for coordinating cancer prevention services and providing support to uninsured individuals in Lower Manhattan. This role involves outreach, education, and case management to ensure clients receive necessary screenings and follow-up care. The coordinator will work closely with healthcare providers and community partners to facilitate access to services and improve health outcomes for vulnerable populations.

Requirements

  • BA/BSW, MA/MSW, MPH and/or RN/Nurse Practitioner in Human Services related field.
  • 2 or more years of program coordination and/or supervisory experience preferred.
  • Interviewing, assessment, engagement and outreach skills; empathy for low-income/vulnerable populations.
  • Ability to work collaboratively and/or independently with other program staff.
  • Case management experience and comprehensive understanding of social determinants of health (SDOH).
  • Experience with managing database(s) including data entry, tracking and reporting.
  • Basic knowledge of cancer disease prevention, staging, and treatment options.
  • Strong leadership skills; able to provide educational, administrative and supportive supervision.
  • Solid writing/grammar/typing and organizational/time management skills.

  • Bilingual in English/Spanish or Chinese preferred.

Responsibilities

  • Work as an integral member of the healthcare team to identify eligible patients and educate them on available clinical and programmatic services.
  • Assess social determinants of health (SDOH) and provide referrals to appropriate services within the program and the local community.
  • Serve as the primary point of contact between the Henry Street team and the Hospital Manhattan Cancer Screening Program (MCSP).
  • Participate in all MCSP staff and contractor meetings to ensure alignment with program goals, updates, and tasks.
  • Oversee and supervise the Patient Navigator's daily tasks, ensuring workflow efficiency and completion of assigned duties.
  • Schedule appointments for necessary cancer screenings, send appointment reminders, and ensure that patients attend scheduled screenings.
  • Submit completed screening forms to the designated MCSP contact, ensuring timely and accurate reporting.
  • Work closely with healthcare providers and the MCSP Case Manager to address and follow up on abnormal screening results.
  • Facilitate appointments for diagnostic services within the contracted clinical teams, ensuring timely reminders and follow-up.
  • Provide support for ad hoc activities and tasks as required by the Hospital Program Manager.
