The Nutritionist provides Home Care visits to patients with nutritional needs and serves as a resource to staff on specific patient nutritional issues related to the overall medical plan of care. Job Description: Essential Duties & Responsibilities including but not limited to: Making home visits to clients in designated geographic territories. Provide direct nutrition services in the home when prescribed by a physician for patients. Planning, organizing, and conducting dietetic education for our staff, clients, and caregivers. Assessing, developing, and counseling clients and families on medical nutrition therapy for preventative care and disease management. Providing reliable, objective information and education to individuals and families in a home health care setting. Translating the latest scientific findings and information so that it is easy for our staff, clients, and caregivers to understand and put into practice. Thorough documentation of clinical treatment through use of electronic medical records. Ability to function as a member of the home health care team with expertise in nutritional care and provide consultation and guidance on personal nutrition or dietary problems to staff. Perform other duties as assigned. Organizational Requirements: Maintain strict adherence to the Continuing Care Confidentiality policy. Participate in departmental and/or interdepartmental quality improvement activities. Participate in and successfully completes Mandatory Education. Perform all other duties as needed or directed to meet the needs of the department. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
