About The Position

Under general supervision will conduct nutrition screening assessment and counseling a diverse population on various nutrition related diseases such as hypertension high cholesterol obesity diabetes as well as implement nutrition care plans patients with diabetes pre diabetes gestational diabetes and other high risk pregnancies hypertension obesity and hyperlipidemia Assist in curriculum educational development and implementation of material development teaching classes and quality improvement work

Requirements

  • Requires completion of Dietetic and Food Administration Program from an accredited university; RD or RD eligible plus one year experience working in a community health care setting
  • Must hold a current BLS Basic Life Support Certificate You will have 30 day grace period to obtain your certificate from the date of hire
  • Knowledge of contemporary principles and practices of nutrition nutritional value of foods interpretation and implementation of special diets and meal planning and preparation that is culturally appropriate
  • Knowledge and experience with Motivational Interviewing Stages of Change Division of Responsibilities
  • Interpretation of anthropometric biochemical clinical and dietary information and determination of appropriate dietary advice that is culturally appropriate
  • Knowledge of aspects of storing andor preparing foods properly; interpretation and implementation of special diets; scopes functions and responsibilities of local health departments
  • Ability to develop implement and evaluate nutrition education activities and coordinate them with other departmental programs
  • Ability to work across cultures and demonstrate support of diversity equity and inclusion
  • Bilingual EnglishSpanish required

Responsibilities

  • Conducts nutrition screening assessment and counseling and implements nutrition care plans for low and high risk patients with diabetes pre diabetes gestational diabetes and other high risk pregnancies hypertension obesity hyperlipidemia
  • Provides individual nutrition and health counseling for those with overweight diabetes hypertension and dyslipidemia
  • Maintains documentation by charting SOAP notes in the progress notes
  • May develop and participation group nutrition education programs and medical group visits ie diabetes child obesity dyslipidemia hypertension
  • Collects and reviews material for education such as pamphlets audio visual aids books and handouts May assist in the development and evaluation of culturally relevant nutrition education materials
  • Assists in data entry and retrieval and other quality improvement efforts for the department
  • Promotes site based employee wellness efforts
  • May provide in service nutritionhealth education to staff and community partners
  • Participates in meetings related to nutrition clinical health education and wellness as requiredassigned
  • Other duties as assigned by supervisor
