About The Position

The Nursing Outcomes Improvement Facilitator - Admin RN l eads interprofessional efforts to implement evidence-based practice standards that advance nursing practice and improve patient outcomes. Responsible for developing nurse leaders through mentorship of nursing colleagues and role modeling of professional practice. Is a resource person and consultant for initiatives involving nursing colleague development, patient care, and nursing practice. Plays a key role in efforts to Pathways designation and support a shared decision-making philosophy. Operationalizes duties and responsibilities in a manner that supports the goals of TMH and the Nursing Division. Embodies the Memorial Health Values of Safety, Integrity, Quality, and Stewardship that support our mission and vision.

Requirements

  • BSN required.
  • Current licensure as an R.N. in the State of Illinois.
  • Holds nursing certification in area of clinical specialty, or acquires within one year.
  • Minimum three years of experience as a staff nurse in acute care required.
  • Minimum two years of demonstrated participation in nursing performance improvement activities, including leading change initiatives, required.
  • Ability to develop, implement and evaluate teaching - learning activities.
  • Demonstrated consistent pattern of continuing professional development.
  • Competent in Microsoft Office
  • Attends Joint Commission, OSHA and Director-mandated inservices.
  • Maintains professional certification.
  • Completion of annual self-evaluation and professional development plan.
  • Participation in nursing leadership peer feedback process.
  • Membership in at least one professional organization.
  • Submits to Director an annual performance appraisal with sufficient documentation to validate credentialing requirements.

Nice To Haves

  • Master’s degree preferred in nursing or other clinically-related discipline.
  • Prior knowledge and involvement in Magnet/Pathways designation processes preferred.

Responsibilities

  • Promote evidence-based practice among nursing colleagues.
  • Observes current nursing practice and compares with recognized professional standards to determine improvement opportunities requiring process changes or educational intervention.
  • Collects and analyzes data to determine necessary process changes and developmental needs of nursing personnel.
  • Develops and leads follow-up interventions to address needs.
  • Facilitates the development of critical thinking skills of nursing colleagues through ongoing consultation, role modeling, and case reviews.
  • Facilitates implementation of Professional Practice Model and Lean Six Sigma tools to promote efficient and effective nursing practice and optimal patient outcomes.
  • Collaborates with interprofessional colleagues and physicians in performance improvement.
  • Facilitates achievement of Nursing Division quality improvement goals.
  • Leads (or co-leads) housewide nursing councils, committees and task forces focused on nursing practice and patient outcomes improvement.
  • Participates in Unit-Based Councils on a regular basis to promote process improvement and sustain practice changes.
  • Participates in research, publication, and poster presentations to generate and disseminate new nursing knowledge and innovations in practice.
  • Serves as a mentor for nursing colleagues, providing for individualized counseling and guidance as needed.
  • Serves as a clinical role model to colleagues.
  • Serves as a resource person and consultant for nursing colleagues to promote effective transition processes.
  • Communicates concerns about ineffective transition processes to the appropriate nurse manager.
  • Collaborates with nurse managers and colleagues to share new knowledge and innovations.
  • Supports interprofessional development activities through shared decision-making.
  • Develops colleagues’ professionalism through coaching, consultation and providing positive feedback.
  • Keeps own knowledge and skills current through review of professional literature and clinical practice opportunities.
  • Provides nursing career coaching to colleagues and students as appropriate.
  • Models the MHS Behavioral Standards.
  • Demonstrates trust and willingness to clarify accountabilities with others which promote team effectiveness.
  • Models constructive verbal, nonverbal, and written feedback to colleagues regarding behaviors not supportive of standards of care and practice.
  • Demonstrates effective group process skills to promote cohesiveness and collaboration within the hospital system.
  • Implements strategies that enhance customer satisfaction and experiences.
  • Performs other related work as required or requested.
