Mary Wade is seeking experienced LPNs for our Skilled Nursing Unit (Long Term Care & Short Term Rehab) Current Openings: Full-Time & Part-Time: All Shifts including every other weekend Hourly Rates Ranges: $32.00 to $36.00 (commensurate with experience) At Mary Wade you will experience a high quality professional culture focused on resident needs. We have 94 beds, 3 shifts, every other weekend rotations and shift differentials. Additional training and support is always available. Requirements

  • Current Connecticut LPN License
  • Minimum of 1 year experience in a Long Term Care Skilled Nursing Facility
  • Flexibility with schedule
  • Ability to work every other weekends

Responsibilities

  • Oversee overall operation and optimal quality of care for the residents on the assigned unit.
  • Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing Policies and Procedures.
  • Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift.
  • Reviews written daily assignments for each resident on assigned unit.
  • Collaborates with Nursing Supervisor to develop and maintain nursing care plans and update these plans as needed
  • Assumes responsibility in supporting the Unit Manager, DCS, ADCS in promoting and maintaining positive employee relations and effective communications among staff
  • Displays a polite and courteous manner to all residents and visitors.
  • Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering.
  • Participates in the annual evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance.
  • Assumes independent responsibility for documenting disciplinary concerns for nursing assistants on designated shifts.
