Requirements

• Current LPN license or GPN permit in Pennsylvania.
• Strong communication and interpersonal skills.
• Ability to stay organized and focused in a dynamic environment.

Nice To Haves

• Experience with EHR systems and basic technology tools (preferred).

Responsibilities

• Provide high-quality, person-centered nursing care to residents.
• Lead and support nursing team members on your assigned neighborhood and shift.
• Observe, assess, and document resident conditions while ensuring families and physicians are kept informed.
• Oversee and implement individualized care plans from admission through ongoing updates.
• Foster a safe, positive, and relationship-centered environment for residents and staff.
• Coordinate appointments, transportation, and interdepartmental communication.
• Support team accountability, including initiating disciplinary procedures when needed.
