About The Position

PRIMARY FUNCTIONS: Leads daily nursing operations. Resource for residents, family and staff with questions or concerns regarding resident care that cannot be answered by the direct service staff. Collaborates closely with the Director of Nursing and Resident Life Coordinator to ensure that resident needs and preferences for clinical care and a meaningful daily routine are integrated and met, and a culture of engagement is nurtured and upheld. Also included in this position is the expectation that the person in this job will: Uphold and put into daily practice the values of Carol Woods. Support the well-being and quality of life of every individual resident. Function as a good steward of resident resources. Contribute to Carol Woods approach to continual quality improvement. Support the concepts of a learning organization.

Requirements

  • Education: Successful completion of an accredited Nursing Program
  • Current, unencumbered license as a Nurse in North Carolina
  • Current certification in CPR – Adult Module (American Health Association or American Red Cross).
  • Requirements: Maintain technical skills related to medication administration, as well as all skills needed to provide hands-on care as a Registered Nurse, particularly for infrequently performed procedures to provide technical assistance to staff as needed.
  • Awareness of Nursing Home Regulations
  • Attend courses/classes as needed to enhance the needed Knowledge, Skills, and Abilities for the position.
  • 2-3 years’ Nursing experience
  • 1-2 years’ supervisory experience
  • Able to: Bend, stoop, squat, crawl reach, kneel and lift to assist residents using proper body mechanics for sustained periods of time.
  • Stand, walk for long periods.
  • Move equipment or small pieces of furniture.
  • Competencies: Communication Confidentiality Judgement Organization Problem Solving/Decision Making Technical/Function Expertise

  • Long-term care experience preferred.

Responsibilities

  • Coordination/assistance to maintain quality documentation by the nursing staff.
  • Collaboration with primary provider (or designee) and nursing staff to clarify orders and concerns about physician assessments and findings, diagnostic/lab values, etc.
  • Ensures completion of nursing assessments including Admissions assessments and quarterly risk management assessments including but not limited to Braden’s, AIMS and Falls Risk.
  • Order medications as needed.
  • Works closely with Resident Life Coordinator to ensure the nursing aspects of the plan of care.
  • Walking rounds and oversight of hands-on care with a special emphasis on residents with the highest acuity and at end-of-life to ensure the highest quality of care.
  • In the moment education for staff Collaboration with nursing staff to provide specific health care promotion/prevention/education to the resident/family at and prior to discharge.
  • As a back-up to the Resident Life Coordinator, coordinates the resident daily schedules with staff for off campus appointments, personal care, family visits, social events, transportation, etc.
  • Participation in development, implementation, and evaluation of resident care standards.
  • Initial contact for any resident/family concern or need, ensuring attention and/or follow-up.
  • Communicates with family members regarding residents' condition (as indicated).
  • Serves as a resource for staff when questions regarding service or care interactions arise.
  • Make daily contact with residents to ensure needs and goals as identified in each resident’s care and service plan are being implemented and met.
  • Ensure employees function according to respective job descriptions.
  • Reviews daily work assignments to assure the right staff mix with resident needs; adjusting as required for care/service delivery.
  • Facilitates an environment of positivity and calm for employees through informal meetings, acknowledgment of contributions, etc.
  • Demonstrates leadership in the areas of proper equipment use and maintenance, ordering and use of expendable supplies, e.g., identifying ways of saving or extending the life of equipment or supplies through proper use.
  • Preparation and involvement in Annual evaluations and necessary Disciplinary actions for Nurses and Resident Life Specialists as needed.
  • Fiscal accountability, proper supply ordering and proper use/care of equipment Completion of mandatory On-line educational modules (monthly and as otherwise assigned).
  • Attendance at required educational sessions, in-services, and staff meetings.
  • Attendance at annual Culture Day session (Full-time mandatory) Involvement/understanding of Organizational Quality Assurance Performance Improvement (QAPI) program.
  • On call responsibility evenings, weekends, and holidays in rotation of all Nurse Coordinators.
  • All other duties as assigned.
