About The Position

Our Lady of Consolation is a proud member of Catholic Health and a recognized leader in providing exceptional quality driven care. We offer Long Term, Post-Acute and Skilled Nursing services. Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. Our nurses, physicians and support staff are devoted to providing patient driven health care in an environment of compassion to our patients, their families and our community. The Nursing Care Coordinator is responsible for coordinating all direct nursing services to patients/residents in collaboration with the unit nursing staff. The Nursing Care Coordinator functions as a geriatric nursing specialist, and resource to the staff on the units and to other institutional departments as necessary.

Requirements

• Graduate of an accredited School of Nursing Program or an accredited College RN degree.
• Minimum of three (3) years experience in long term care or a combination of long-term care and acute care.
• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long term care.
• Licensed to practice as a Registered Nurse in New York State.
