This position is responsible for assisting the needs of the Home Care Coordinator. Key duties include communication with Personal Care Contracted Companies and DME Contracted Companies. The role involves assisting in setting up Personal Care Services in the home, as well as ordering and tracking Durable Medical Equipment (DME) and supplies. Additionally, the Nursing Assistant assists in coordinating care between families, personal care companies, and transportation services. The role also requires performing telephone audits with participants who are receiving personal care in their homes.
