This position will primarily work in the SW region of the Twin Cities. Must provide own transportation, mileage reimbursement available. No weekends required, Monday-Friday 8:00 am -4:30 pm. As an Optage caregiver, you will be serving clients in various locations in the Minnesota metro area, providing individualized one on one care. Schedules are designed to minimize the amount of traveling you will be doing. Requirements

  • Successful completion of a state approved nursing assistant training program and current certification in state(s) in which he/she practices OR successful completion of a home health aide instruction course and successful completion of home health aide competency testing to work in Home Care.
  • Demonstrated competencies in person-directed, elder/resident-directed practice, home building, critical thinking, teamwork, self leadership, and interpersonal effectiveness.
  • Demonstrated compatibility with the PHS mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

  • The Home Health Certified Nursing Assistant is a paraprofessional member of the Interdisciplinary team who works under the supervision of a Registered Nurse and performs various services for the client as assigned to meet the client's personal needs while maintaining consistency with regulations and established best practices.
  • We provide care in the patient’s home which may be a personal residence, group home, assisted living or skilled nursing setting.
