About The Position

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well. We are seeking a Nursing Assistant (CNA/NAR) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for providing resident care and services. This CNA/NAR schedule includes; 32 hours every two weeks ; part-time Day Shift ; 6:00am-2:30pm Block Schedule: Week 1: Monday, Saturday, and Sunday Week 2: Friday Transitional Care Unit Every other weekend and holiday rotation

Requirements

  • less than 1 year of experience
  • Nursing Assistant Registered on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.

Responsibilities

  • Provides personalized care to residents in accordance with individual care plans, including assistance with bathing, oral hygiene, grooming, toileting, dressing, and showering. Responds promptly to call lights and resident needs.
  • May perform and document vital signs as outlined in the community’s patient care model.
  • Delivers AM, PM, and NOC care routines, including peri-care and catheter care.
  • Assists with turning, lifting, and transferring residents, utilizing mechanical aids or working collaboratively with other staff as needed.
  • Observes and monitors residents’ conditions, reporting changes clearly and accurately to appropriate personnel.
  • Documents care and observations in the electronic medical record system in a timely and thorough manner.
  • Proactively addresses and seeks resolution for resident and family concerns.
  • Participates in shift handoffs by receiving and providing detailed reports to ensure continuity of care.
  • Performs additional duties such as maintaining cleanliness and order in resident rooms and work areas, reporting equipment issues to maintenance, and answering phone calls efficiently.
