About The Position

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well. We are seeking a Nursing Assistant (CNA/NAR) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for providing resident care and services. This CNA/NAR schedule includes; 40 hours every two weeks ; Part Time Evening Shift ; 2:00pm-10:30pm Block Schedule: Week 1: Thursday, Saturday, and Sunday Week 2: Wednesday and Friday Long Term Care Unit Every other weekend and holiday rotation

Requirements

  • less than 1 year
  • Nursing Assistant Registered Registered (NAR) on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.)

Responsibilities

  • Assists residents with personal cares based on individual plan of care:
  • Provides AM/PM cares
  • Provides full bath/shower weekly
  • Provides oral hygiene per care plan
  • Provides nail care, shaving as appropriate
  • Checks hearing aids; cleans eyeglasses daily
  • Assures that residents are clean, dry, and dressed appropriately
  • Provides offloading for residents
  • Performs Restorative Nursing Programs per facility policy.
  • Provides adequate nutrition & hydration (distributes meal trays, assists residents as needed, documents intake/output meal consumption, provides nourishment & water between meals as ordered)
  • Assures individual toileting plan is followed
  • Answers call lights promptly
  • Observes residents and reports status to appropriate staff:
  • Receives report at the start of the shift
  • Gives report to team leader at the end of the shift & PRN
  • Notifies appropriate staff of any changes in a timely & appropriate manner (i.e. incidents, skin integrity, behavior changes, illness)
  • Offers pertinent observations & input for documentation and care plan updates
  • Utilizes Nursing Assistant Kardex or Care sheet
  • Takes and documents weights as ordered
  • Documents resident cares promptly in EHR
  • Responds to emergency situations appropriately
  • Handles resident and family complaints tactfully and refers concerns to appropriate staff members
  • Follows Ebenezer policies and procedures
  • Reports resident change of condition to Licensed Nurse
  • Attends mandatory, annual, and any other educational opportunities on site.
  • Performs environmental and other responsibilities as assigned:
  • Maintains order & cleanliness in resident rooms
  • Maintains order & cleanliness in other work areas
  • Reports malfunctioning equipment to Maintenance Department
  • Assists with admissions, transfers & discharges as needed
  • Answers phones with proper customer service etiquette
  • Performs other duties as assigned and directed
