As a Nursing Admission Specialist, you will initiate and/or resume home care for patients of all ages and health backgrounds in the east region, including the Eastern Cuyahoga, Lake, Geauga, and Ashtabula Counties. This involves offering comprehensive assessments, care plans, and ongoing support to meet their unique needs. Your efforts help patients stay independent and confident in managing their own care, ultimately reducing hospital readmissions and poor outcomes. This position offers rewarding experiences and multiple resources for enhancing nursing skills, education, and professional development.

Requirements

  • Graduate of an accredited school of Professional Nursing
  • Knowledge of Home Care and the OASIS data set
  • Experience with Infusion therapy, venipuncture, diabetes management, wound/ostomy care, urinary catheterization, enteral feeding, Oxygen Therapy, administration of injectable medication and Left Ventricular Assistive Devices
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
  • Valid Driver's License (VDL) upon hire and per policy 8-175 thereafter
  • Proof of automobile $100,000/$3,000,000 insurance coverage and per policy 8-175 thereafter
  • One year of current clinical experience as an RN
  • One year of experience in an acute medical/surgical or home care setting

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN)
  • Six months of home health care experience

Responsibilities

  • Initiate and/or resume home care for patients of all ages and health backgrounds in the east region, including the Eastern Cuyahoga, Lake, Geauga and Ashtabula Counties, offering comprehensive assessments, care plans and ongoing support to meet their unique needs
  • Conduct and submit a complete and accurate Comprehensive Assessment in accordance with OASIS reporting requirements and agency policies and procedures
  • Identify immediate care and support needs and obtain orders to address psychosocial and treatment priorities
  • Make appropriate admission decisions based on patient eligibility, acuity and safety
  • Obtain consent for care
  • Demonstrate knowledge of coverage criteria based on payer source, including: skilled needs, intermittent care, homebound status, and physician ordered care
  • Manage own patient assignment and make critical decisions in the field
  • Treat patients in the context of their home environment, allowing for truly personalized care
  • Deliver efficient, high-quality care in the home setting
